    Notice given that the Administrative Law Judge has completed a review of the application filed by Texzon Utilities, Ltd. ("Applicant") on September 24, 2012 and hereby rules that the information described below shall be filed with the Chief Clerk of the Illinois Commerce Commission on or before October 5, 2012. The response to this ruling must include (1) the docket number, case caption, and an original signature, (2) a verification page, and (3) a certificate of service indicating that the response was served on all those on the service list. The response to this ruling must be actually received by the date indicated, regardless of whether the response is filed with the Chief Clerk as hardcopy or electronically through e-Docket. Applicant shall also electronically serve its response on the Administrative Law Judge (jalbers@icc.illinois.gov) by the date indicated. 1) 83 Ill. Adm. Code 454.60(c) states, “[the] applicant shall include in its application an exhibit containing a corporate organizational chart and indicating the position of persons indicated in subsection (a) of [Section 454.60].” Applicant indicates in paragraph 9 of the application that Attachment C contains a corporate organizational chart. Attachment C, however, simply lists five of Applicant’s employees, along with brief background information. Please provide an actual organizational chart setting forth Applicant’s reporting hierarchy and identifying the five individuals by at least name and job title. 2) The license and permit bond attached to the application states, “WHEREAS, the Principal has obtained, or is about to obtain a license or permit for Utility Provider.” Please provide a new license or permit bond or rider to the filed bond referencing the appropriate statutory provisions and code part, as set forth below and as set forth in the “ABC Sample License or Permit Bond” on the Commission’s website. THE CONDITIONS OF THIS OBLIGATION ARE SUCH, That WHEREAS, the Principal has been or is about to be granted a license or permit to do business to operate as an ABC (Agent, Broker, or Consultant) under 220 ILCS 5/16-115C and is required to execute this bond under 83 Illinois Administrative Code Part 454.80 by the Obligee.
