  Notice given that the Administrative Law Judge has completed a review of the application filed by Power2Switch, LLC ("Applicant") on March 2, 2012 and hereby rules that in light of the change in corporate structure, Applicant must file an amended application in the name of Power2Switch, Inc. with the Chief Clerk of the Illinois Commerce Commission on or before April 2, 2012. The amended application should follow the “ABC Application Form” and “ABC Sample License or Permit Bond” form on the Commission’s website. The amended application must include (1) the docket number, case caption, and an original signature, (2) a verification page, and (3) a certificate of service indicating that the response was served on all those on the service list. The amended application must be actually received by the date indicated, regardless of whether the response is filed with the Chief Clerk electronically or as hardcopy. Applicant shall also electronically serve a Word version of its amended application on the Administrative Law Judge (jalbers@icc.illinois.gov) by the date indicated. In the alternative, Applicant may file a motion to withdraw its March 2, 2012 filing and submit a new application in the name of Power2Switch, Inc. at its convenience using the “ABC Application Form” and “ABC Sample License or Permit Bond” form on the Commission’s website.   Notice given that the Administrative Law Judge has completed a review of the amended application filed by Power2Switch, Inc. ("Applicant") on April 11, 2012 and hereby rules that the information described below shall be filed with the Chief Clerk of the Illinois Commerce Commission (“Commission”) on or before April 20, 2012. The response to this ruling must include (1) the docket number, case caption, and an original signature, (2) a verification page, and (3) a certificate of service indicating that the response was served on all those on the service list. The response to this ruling must be actually received by the Chief Clerk by the date indicated, regardless of whether the response is filed with the Chief Clerk electronically or as hardcopy. Applicant shall also electronically serve a Word version of its response on the Administrative Law Judge (jalbers@icc.illinois.gov) by the date indicated. General Requirements 1) Paragraph 7 of the application form directs an applicant to disclose whether it has had any complaints filed against it for its provision of any services in the electric or gas industry in this or any other jurisdiction. Applicant has not provided any response concerning complaints. Please respond fully to Paragraph 7. Managerial Requirements 2) Section 454.60(c) of Part 454 states, “[the] applicant shall include in its application an exhibit containing a corporate organizational chart and indicating the position of persons indicated in subsection (a) of [Section 454.60].” The organizational chart Applicant provided with the amended application does not identify any of the individuals filling the various positions. Please provide a corporate organization chart identifying by name those individuals filling the various positions in compliance with Section 454.60(c). Parties advised electronically.
