    Notice given that the Administrative Law Judge has completed a review of the application filed by NFuse Direct, LLC ("Applicant") on November 28, 2012 and hereby rules that the information described below shall be filed with the Chief Clerk of the Illinois Commerce Commission on or before December 21, 2012. The response to this ruling must include (1) the docket number, case caption, and an original signature, (2) a verification page, and (3) a certificate of service indicating that the response was served on all those on the service list. The response to this ruling must be actually received by the date indicated, regardless of whether the response is filed with the Chief Clerk as hardcopy or electronically through e-Docket. Applicant shall also electronically serve its response on the Administrative Law Judge (jalbers@icc.illinois.gov) by the date indicated. 1) As evidence of being authorized to conduct business in Illinois, Applicant has provided documentation from the Illinois Secretary of State for C4 Connections, LLC. The Secretary of State website has no record of NFuse Direct, LLC. In addition, the license or permit bond provided by Applicant identifies C4 Connections, LLC as the principal. While paragraph 2 of the application form indicates that Applicant does business as C4 Connections, LLC, the Commission can not issue an ABC license in the name of an entity not recognized by the Secretary of State. To remedy this shortcoming, Applicant can EITHER (A) register NFuse Direct, LLC with the Illinois Secretary of State and obtain a new license or permit bond identifying the principal as NFuse Direct, LLC (or obtain a rider changing the name on the existing license or permit bond) OR (B) request that the name on its application be changed to C4 Connections, LLC. 2) Applicant answered paragraph 7 of the application form with regard to NFuse Direct, LLC. Please also provide the requested information as it pertains to C4 Connections, LLC. Notice served electronically to parties.
