    Notice given that the Administrative Law Judge has completed a review of the application filed by CQI Associates, LLC ("Applicant") on October 15, 2012 and hereby rules that the information described below shall be filed with the Chief Clerk of the Illinois Commerce Commission on or before November 2, 2012. The response to this ruling must include (1) the docket number, case caption, and an original signature, (2) a verification page, and (3) a certificate of service indicating that the response was served on all those on the service list. The response to this ruling must be actually received by the date indicated, regardless of whether the response is filed with the Chief Clerk as hardcopy or electronically through e-Docket. Applicant shall also electronically serve its response on the Administrative Law Judge (jalbers@icc.illinois.gov) by the date indicated. 83 Ill. Adm. Code 454.80(a) provides as follows: The applicant shall execute and maintain a license or permit bond issued by a qualifying surety or insurance company authorized to transact business in the State of Illinois in favor of the People of the State of Illinois. The amount of the bond shall equal $5,000. The bond shall be conditioned upon the full and faithful performance of all duties and obligations of the applicant as an ABC and shall be valid for a period of not less than one year. The cost of the bond shall be paid by the applicant. The applicant shall file this bond as part of its application for certification. In attempting to comply with this subsection, Applicant appears to have attached copies of three different bonds to its application. None of the bonds fully complies with the Commission’s rules. Please submit a copy of a new license or permit bond (or a copy of a rider to one of the existing bonds) (1) identifying “CQI Associates, LLC” as the principal, (2) identifying “The People of the State of Illinois” as the obligee, and (3) using as the conditions of the obligation the language from the ABC Sample License or Permit Bond from the Commission’s website (see http://www.icc.illinois.gov/Electricity/ABC.aspx). Applicant is encouraged to use the ABC Sample License or Permit Bond in its entirety. Notice served electronically to parties.
